Being told that a semen test shows no sperm at all is one of the hardest pieces of news for a couple trying to conceive, and many men are too embarrassed to talk about it. Azoospermia is more common than people think: it is found in about 1 in 100 men and in many men with infertility. Importantly, there are different types with different causes, and the type decides what can be done. This guide explains azoospermia, the tests needed, the causes, and the homeopathic medicines traditionally considered, along with how our specialists approach each couple with sensitivity.

Azoospermia: quick answers
- What is it? No sperm found in the semen sample on testing, usually confirmed on at least two separate samples after centrifuging.
- Two main types: Obstructive (sperm are made but blocked from leaving) and non-obstructive (sperm production is low or absent).
- Tests: Repeat semen analysis, hormone tests (FSH, LH, testosterone), scrotal ultrasound, genetic tests and sometimes a testicular biopsy.
- Good to know: Some men with azoospermia still have sperm in the testes that can be retrieved for assisted conception.
- Our approach: Meet our specialists for your personal best homeopathic remedy and a plan that supports your recovery and reproductive health.
About azoospermia
A semen sample is called azoospermic when, after careful laboratory examination, no sperm are found. The result is confirmed on at least two samples, because a single result can be affected by an incomplete sample, a recent illness or the way the sample was collected. Azoospermia is diagnosed in about 1 in 100 men and in 10 to 15 out of 100 men being investigated for infertility.
In obstructive azoospermia, sperm are produced normally but cannot reach the semen because of a blockage in the tubes (for example after infection, injury, previous surgery or absence of the vas deferens). Hormone levels and testicle size are usually normal. In non-obstructive azoospermia, the testicles are not producing enough sperm because of hormonal problems, genetic conditions (such as Klinefelter syndrome or Y chromosome microdeletions), undescended testes, past mumps orchitis, chemotherapy, radiation, varicocele or other causes.
The type determines the options. Blockages can sometimes be corrected by surgery. In non-obstructive azoospermia, small pockets of sperm production can sometimes be found by retrieval procedures (such as micro-TESE) and used with assisted conception (ICSI). Our specialists take a detailed history of both partners, review reports, and plan a personal homeopathic approach to support general health, hormonal balance and wellbeing alongside whatever assessment or treatment the couple chooses.
Common symptoms of azoospermia
Symptoms vary from person to person. Common ones include:
- Usually no symptoms; the problem is found when a couple cannot conceive
- Small or soft testicles in some men with non-obstructive azoospermia
- Low semen volume in some types
- Signs of low testosterone such as low sex drive, tiredness and reduced facial hair, in some men
- A history of mumps, undescended testes, hernia surgery, injury, infection or chemotherapy
- Pain or swelling in the testicles or a varicocele in some men
- Normal erections and ejaculation in most men
Causes and risk factors of azoospermia
There is rarely a single cause. Factors that may contribute include:
- Blockage of the sperm ducts after infection, surgery, injury or from birth
- Hormonal problems affecting the pituitary gland or the testicles
- Genetic conditions such as Klinefelter syndrome and Y chromosome microdeletions
- Undescended testicles in childhood and testicular injury or torsion
- Past mumps orchitis, tuberculosis and other infections
- Chemotherapy, radiation and some medicines, including anabolic steroids and testosterone supplements
- Varicocele, heavy heat exposure, smoking, alcohol and recreational drugs, which may lower sperm production
Warning signs and when to get urgent help
Please go to the nearest hospital emergency department immediately if there is:
- Sudden severe pain in one testicle, with swelling, nausea or vomiting (possible torsion)
- Redness, swelling and fever with testicular pain
- A fast-growing or hard lump in the testicle
- Blood in semen or urine that does not settle
- Severe lower abdominal or groin pain with fever
- Injury to the groin with swelling or bruising

The medicines below are commonly considered in homeopathy for azoospermia, with the symptom picture traditionally associated with each. They are listed for general information, not as a prescription.

Self-care and lifestyle tips
These measures support general health and may ease symptoms, but they do not replace treatment when it is needed.
- Repeat the semen analysis at a good laboratory, with 3 to 5 days of abstinence, and collect the sample exactly as instructed.
- Stop smoking, tobacco and recreational drugs, and keep alcohol low.
- Avoid prolonged heat on the testicles from hot baths, saunas, laptops on the lap and very tight underwear.
- Maintain a healthy weight, exercise regularly, sleep well and manage stress.
- Avoid testosterone or anabolic steroid supplements, which can switch off sperm production.
- Eat a balanced diet with fresh fruit, vegetables, nuts, seeds and adequate protein, and drink enough water.
- Keep your reports together (semen analysis, hormones, scans, genetic tests) and bring both partners’ records to your consultation.
Frequently asked questions about azoospermia
1. What is the difference between azoospermia and low sperm count?
In azoospermia there are no sperm in the semen sample. In low sperm count (oligospermia), some sperm are present but fewer than normal.
2. What causes azoospermia?
Blockage in the tubes (obstructive) or reduced sperm production (non-obstructive) due to hormonal, genetic, testicular or lifestyle causes.
3. Is azoospermia always permanent?
Not always. Some causes, such as infections, hormone problems, medicines or varicocele, can improve, while others cannot. A thorough evaluation shows which type it is.
4. Can a man with azoospermia ever father a child?
Many men with azoospermia have sperm in the testes that can be retrieved and used for assisted conception. The options depend on the type and your reports.
5. Can homeopathy help in azoospermia?
Homeopathy is offered as individualised care alongside your assessment. Our specialists study your reports, history and temperament to choose your personal remedy and a plan that supports your recovery and reproductive health.
6. Do semen tests need to be repeated?
Yes. A diagnosis of azoospermia is usually confirmed on at least two samples, collected correctly, with the sample centrifuged and examined.
7. Does azoospermia affect sexual performance?
Usually not. Erections, ejaculation and sexual desire are normally unaffected, unless there is low testosterone.
