Ankylosing spondylitis (AS) is a long-term inflammatory arthritis that mainly affects the spine and the sacroiliac joints at the base of the spine. It typically begins in young adults with lower back pain and morning stiffness that eases with exercise, and over time can stiffen the spine. Early diagnosis and treatment by a rheumatologist help to keep the spine flexible. This page explains the symptoms, causes, tests and treatment of ankylosing spondylitis, and how homeopathic care is offered as individualised, supportive care alongside it.

Ankylosing spondylitis at a glance
- What it is: an inflammatory arthritis of the spine and sacroiliac joints that can cause stiffening of the spine.
- Main symptoms: lower back and buttock pain with morning stiffness that eases with exercise, and tiredness.
- Who gets it: mostly young adults, more often men, often with the HLA-B27 gene.
- Key rule: do not stop prescribed anti-inflammatory or biologic medicines on your own; exercise is part of treatment.
- Our approach: individualised homeopathic care alongside your rheumatologist’s treatment and physiotherapy.
What is ankylosing spondylitis?
In ankylosing spondylitis the immune system causes inflammation where ligaments and tendons attach to bone, particularly in the sacroiliac joints between the spine and pelvis and the vertebrae. Over years the body may try to heal the inflammation by laying down new bone, which can fuse the vertebrae and stiffen the spine. Not everyone gets this, and the course varies widely.
AS can also cause inflammation of the eyes (uveitis), the heels and other joints, and sometimes the bowel, heart and lungs. Early treatment and regular exercise can reduce pain and help preserve movement, which is why diagnosis should not be delayed.
Symptoms of ankylosing spondylitis
Common symptoms include:
- Dull pain in the lower back and buttocks that comes on gradually, often before the age of 40
- Morning stiffness lasting more than 30 minutes, easing with exercise and not with rest
- Pain that wakes you in the second half of the night
- Stiffness and pain in the neck and upper back as the disease spreads upward
- Pain and swelling in the hips, shoulders, knees or heels
- Chest tightness or pain on taking a deep breath, from rib involvement
- Tiredness, poor sleep and loss of appetite
- A red, painful eye with light sensitivity (uveitis)
Causes and risk factors of ankylosing spondylitis
Common causes and risk factors include:
- An inflammatory immune process in the spine, whose exact trigger is unknown
- A strong genetic link, particularly to the HLA-B27 gene
- A family history of AS or related conditions
- Male sex and age under 40 at onset
- Related conditions such as psoriasis and inflammatory bowel disease
- Gut and other infections, which may play a role in susceptible people
- Smoking, which worsens the disease
- Not exercising, which worsens stiffness
How ankylosing spondylitis is diagnosed
Diagnosis combines your history, examination of spinal movement and tests:
- Examination of spinal movement, chest expansion and the sacroiliac joints
- Blood tests such as ESR, CRP and HLA-B27 (a positive result does not prove the disease on its own)
- X-rays of the pelvis and spine, which can show changes in the sacroiliac joints
- MRI of the sacroiliac joints and spine to detect early inflammation before X-rays change
- Eye examination if there is a painful, red eye
Warning signs: when to get urgent help
Please see a doctor promptly, and go to the nearest hospital emergency department if there is:
- A painful, red eye with light sensitivity or blurred vision, which needs urgent eye care
- Severe back pain after even a minor fall, as the spine in AS is more prone to fractures
- Weakness, numbness or loss of bladder or bowel control
- Chest pain or breathlessness
- Back pain with fever, or persistent back pain that wakes you at night and began before the age of 40 (see a doctor soon)
Conventional treatment options
Treatment, directed by a rheumatologist, combines medicine and exercise:
- Regular exercise and physiotherapy, which are the cornerstone of treatment and keep the spine flexible
- Anti-inflammatory painkillers (NSAIDs) for pain and stiffness
- Biologic medicines (such as TNF or IL-17 inhibitors) for people who do not respond to NSAIDs
- Treatment of eye inflammation, and steroid injections into affected joints in selected cases
- Posture advice, smoking cessation and regular review of bone health

Self-care and lifestyle tips
These measures support general health and may ease symptoms, but they do not replace treatment when it is needed.
- Exercise every day: stretching, spinal extension, swimming and walking are especially helpful, and are part of the treatment.
- Keep your posture upright when sitting and standing, and sleep on a firm mattress with a thin pillow.
- Use a warm pack or warm shower before exercise to ease stiffness.
- Stop smoking, which worsens AS and reduces the benefit of treatment.
- Maintain a healthy weight, and keep up regular follow-up with your rheumatologist.
- Ask about bone health and vitamin D, and see an eye doctor promptly for a painful red eye.

Frequently asked questions about ankylosing spondylitis
1. What is the homeopathic treatment for ankylosing spondylitis?
Homeopathic treatment is individualised: the medicine is chosen after detailed case-taking, based on your symptoms, what eases or aggravates the pain and your general health. Rhus toxicodendron, Calcarea fluorica, Bryonia and Kalmia are among those traditionally considered. It is offered as supportive care alongside your rheumatologist’s treatment.
2. What are the early signs of ankylosing spondylitis?
Lower back and buttock pain with morning stiffness that eases with exercise, beginning gradually before the age of 40, and often waking you in the early morning hours.
3. Does a positive HLA-B27 mean I have AS?
Not necessarily. Many people with HLA-B27 never develop AS, and some people with AS do not have it. Doctors use it together with symptoms and imaging.
4. Does ankylosing spondylitis always cause a bent spine?
No. Many people keep good movement, particularly with early treatment and regular exercise.
5. Can I consult Welling Homeopathy online for ankylosing spondylitis?
Yes. You can share your reports and consult our specialists online from anywhere. We have been consulting patients online for 25 years.
6. Which is the best homeopathic medicine for ankylosing spondylitis?
There is no single best homeopathic medicine for ankylosing spondylitis. Rhus toxicodendron, Calcarea fluorica, Bryonia, Kalmia and Causticum are among the medicines traditionally considered, but the right medicine depends on your symptoms, what brings them on or eases them, your general health and your personality. At Welling Homeopathy our specialists choose it after a detailed case-taking, so please consult them rather than choosing a medicine yourself.
7. How long does homeopathic treatment for ankylosing spondylitis take?
The duration depends on how long you have had ankylosing spondylitis, how severe it is, what treatment you have already taken and your general health. Some people notice a change in their symptoms within weeks, while others need several months of regular follow-up. Your specialist will explain what to expect at your first consultation and review your progress at each follow-up.
8. Is it safe to take homeopathic medicines along with my current medicines for ankylosing spondylitis?
Homeopathic medicines are highly diluted and are not known to interfere with other medicines, but you should always tell your doctor and our specialists about everything you take. Please do not stop, skip or change any prescribed medicine on your own; any change should be discussed with the doctor who prescribed it.
9. What are the early signs of ankylosing spondylitis?
Dull lower back and buttock pain that begins gradually before the age of 40, with morning stiffness that eases with exercise and wakes you in the early morning, are the usual early signs of ankylosing spondylitis.
10. What should I eat or avoid if I have ankylosing spondylitis?
Eat a balanced diet, keep a healthy weight and do not smoke. No special diet treats ankylosing spondylitis, but good nutrition and daily exercise support your spine and general health.
11. What is the difference between conventional and homeopathic treatment of ankylosing spondylitis?
Conventional treatment of ankylosing spondylitis uses regular exercise and physiotherapy, anti-inflammatory painkillers and, for those who do not respond, biologic medicines. Homeopathic treatment, in contrast, is individualised: the medicine is chosen for you as a whole person, after detailed case-taking, and is offered alongside the conventional care that your doctor advises.
12. Who is the best homeopathy doctor for ankylosing spondylitis in Mumbai?
