Bipolar disorder is a mood condition in which a person swings between periods of unusually high or irritable mood (mania or hypomania) and periods of depression. The shifts are more intense than ordinary ups and downs, and can affect sleep, energy, behaviour, work and relationships.
Symptoms of mania
- Very high energy and a greatly reduced need for sleep
- Racing thoughts and rapid, pressured speech
- Inflated self-confidence or unrealistic plans
- Impulsive spending, risk-taking or reckless behaviour
- Marked irritability and difficulty concentrating
What is hypomania?
Hypomania is a milder form of the high phase. The person may feel unusually productive and cheerful, but the change is noticeable to others, and it can progress to full mania.
Symptoms of depression in bipolar disorder
Persistent sadness or emptiness, loss of interest, tiredness, sleeping too much or too little, changes in appetite, guilt, difficulty concentrating and, in some, thoughts of death or suicide.
Psychosis and mixed states
In severe episodes a person may hold false beliefs or hear or see things that are not there. In a mixed state, symptoms of mania and depression occur together, which is a particularly high-risk period.
Triggers
There is often no clear cause, but episodes can be triggered by major life changes, sleep loss, stress, substance use and some medicines such as antidepressants taken without a mood stabiliser.
Treatment
Bipolar disorder needs proper psychiatric assessment. Treatment usually combines a mood-stabilising medicine with therapy, regular sleep and routine, and avoidance of alcohol and drugs.
