Frozen shoulder (adhesive capsulitis) is a condition in which the shoulder becomes painful and stiff because the capsule around the joint thickens and tightens. It is common between 40 and 60 years of age, is more frequent in people with diabetes or thyroid disease, and can follow an injury or a long period of immobility.
The three stages
- Freezing: increasing pain, worse at night, with a gradual loss of movement (about 2–9 months)
- Frozen: pain eases but stiffness is marked, and everyday tasks such as combing the hair or reaching behind the back are hard
- Thawing: movement slowly returns over months
The whole course can last one to three years, though earlier treatment can reduce suffering.
Treatment
- Gentle stretching and physiotherapy, which are the mainstay
- Pain relief, heat, and in some cases steroid injections
- Control of diabetes and thyroid disease
- Procedures under anaesthesia are considered only for stubborn cases
