White lung syndrome is not a formal medical diagnosis but a media term for pneumonia in which a chest X-ray shows the lungs looking white, because they are filled with inflammation and fluid. It gained attention after a rise in childhood pneumonia cases in China, Denmark and other countries, many linked to the bacterium Mycoplasma pneumoniae. This page explains what white lung syndrome means, its symptoms and causes, who is at risk, how it is diagnosed and treated, how to prevent it, and when to see a doctor.

White lung syndrome at a glance
- What it is: a popular name for pneumonia, mostly in children, with a very white appearance of the lungs on chest imaging; not a separate disease.
- Usual cause: Mycoplasma pneumoniae (“walking pneumonia”), viruses such as influenza, RSV and COVID-19, or other bacteria.
- Symptoms: persistent cough, fever, tiredness, chest discomfort and breathlessness.
- Treatment: antibiotics for bacterial causes, supportive care and, if needed, hospital oxygen.
- See a doctor if: breathing is fast or difficult, the fever lasts more than 3 days, or a child is drowsy or not drinking.
What is white lung syndrome?
On a normal chest X-ray the lungs look dark because they are full of air. When the air sacs fill with fluid, pus and inflammatory cells, as in pneumonia or acute respiratory distress syndrome (ARDS), more of the lung looks white. “White lung” describes this appearance. It has been used in news reports for widespread childhood pneumonia, and the term has no formal medical definition.
The illness drew worldwide anxiety as cases rose across continents, from the US to Europe and China, with pneumonia-like symptoms and epidemic proportions in countries such as Denmark. Experts pointed out that a rise was not unexpected, because there were relatively few infections in the previous three years of pandemic restrictions, leaving children with less immunity.
Symptoms
- A cough that lingers for weeks and may be dry at first
- Fever, which may be low-grade in mycoplasma pneumonia
- Tiredness, sore throat and headache
- Chest discomfort and wheezing
- Fast breathing or breathlessness
- Poor feeding or irritability in young children
- Bluish lips or low oxygen in severe cases
Causes: what can lead to white lungs?
| Cause | Notes |
|---|---|
| Mycoplasma pneumoniae | A common bacterial cause of “walking pneumonia” in school-age children; usually mild, with a lingering cough; treated with macrolide antibiotics |
| Respiratory viruses | Influenza, RSV, COVID-19 and adenovirus can cause pneumonia and ARDS |
| Other bacteria | Streptococcus pneumoniae and others cause more acute pneumonia |
| Aspiration, toxins or inflammation | Inhaling fumes, vomit or chemicals; autoimmune or other lung disease |
Who is at risk?
- Children, especially school-age, after a period of low exposure to germs
- Infants, older adults and people with weak immunity, asthma, heart or lung disease
- Unvaccinated people, and those exposed to smoke and air pollution
- Crowded settings such as schools, hostels and care homes
How it is diagnosed and treated
- Examination, oxygen saturation and chest X-ray or CT scan
- Blood tests and swabs, to identify the germ
- Antibiotics such as azithromycin or other macrolides for mycoplasma and bacterial pneumonia; antibiotics do not help viral pneumonia
- Rest, fluids, fever control and oxygen in hospital when oxygen levels are low
- Intensive care for ARDS or severe respiratory failure
Prevention
- Keep up to date with influenza, COVID-19, pneumococcal and other recommended vaccines
- Wash hands often, cover coughs and stay at home when unwell
- Keep children home from school with fever
- Avoid smoke and pollution; improve indoor ventilation
- Seek early medical care for a persistent cough or breathing trouble
When to see a doctor
See a doctor promptly if a cough lasts more than two weeks, or fever lasts more than three days. Go to hospital immediately for fast or laboured breathing, chest indrawing, blue lips, drowsiness, refusal to feed or drink, or a child who looks very unwell.
Where homeopathic care fits
Homeopathic care can be offered as supportive treatment for lingering cough and weakness after a chest infection, alongside medical care. It is not a substitute for antibiotics, oxygen or hospital treatment. See also bronchitis and asthma.
Frequently asked questions about white lung syndrome
1. What is white lung syndrome?
It is a popular term for pneumonia, mostly in children, in which the lungs look white on a chest X-ray. It is not a separate disease.
2. What causes white lung syndrome?
Mostly Mycoplasma pneumoniae, along with viruses such as influenza, RSV and COVID-19 and other bacteria.
3. Is white lung syndrome contagious?
The germs that cause pneumonia can spread through coughs and sneezes, so hygiene and staying home when ill help.
4. What are the symptoms of white lung syndrome?
A persistent cough, fever, tiredness, chest discomfort and breathlessness, and fast breathing in children.
5. How is white lung syndrome treated?
Treatment depends on the cause: antibiotics for bacterial pneumonia, supportive care, and hospital oxygen for severe cases.
6. Is walking pneumonia the same as white lung syndrome?
Walking pneumonia is a mild pneumonia, often caused by mycoplasma. The media called the severe childhood cases “white lung”; the two terms overlap but are not identical.
7. When should I take my child to hospital?
When the child has fast or difficult breathing, blue lips, drowsiness, is not drinking, or has a high fever for more than three days.
8. How can I prevent white lung syndrome?
With vaccination, hand washing, avoiding sick contacts, good ventilation and early medical care for lingering cough.
