Lichen planus is a fairly common inflammatory condition that can develop on one or several parts of the body. It may appear on the skin, inside the mouth, or in both places, and it can also change the look of the nails, scalp and genitals.
The different forms
- Skin (cutaneous): itchy, flat-topped, purple or reddish-purple bumps, often on the wrists, ankles, shins and lower back; fine white lines (Wickham’s striae) may be visible on the surface
- Oral: white lacy patches or red, sore areas on the inner cheeks, gums or tongue
- Nails: ridging, thinning, splitting or loss of the nail
- Scalp (lichen planopilaris): redness and scarring with hair loss
- Genital: itchy or sore patches on the genitals
What causes it?
It is thought to be an immune-mediated reaction in which the body’s own cells attack the skin or mucous membrane. It is not contagious. Triggers can include hepatitis C infection, certain medicines, dental materials, stress and, in some people, no identifiable cause.
Diagnosis and course
Diagnosis is usually based on appearance, and a small biopsy confirms it when needed. Skin lichen planus often settles within one to two years, while oral and genital forms tend to last longer. Persistent mouth sores need regular review by a doctor, because long-standing oral lichen planus warrants monitoring.
