A child goes to bed with a slight cold and, in the middle of the night, wakes with a harsh, barking cough like a seal and a noisy, squeaky sound when breathing in. This is croup, a common viral infection in young children that is frightening to hear but usually mild. In a small number of children it can narrow the airway enough to need urgent treatment. This guide explains how to recognise croup, when to rush to hospital, how to care for a child at home, and the homeopathic medicines traditionally considered.

Croup: quick answers

  • What is it? A viral infection that swells the voice box and windpipe, causing a barking cough, hoarse voice and a noisy sound on breathing in (stridor).
  • Who gets it? Mostly children between six months and three years, often in the cooler months, and usually worse at night.
  • How long does it last? Typically two to three days, with the worst on the second night; the cough can linger for up to a week.
  • Emergency signs: Stridor when calm or at rest, drawing in of the skin between the ribs or at the neck, blue lips, drooling or severe distress.
  • Homeopathy: Offered as supportive care; it does not replace medical care when the breathing is affected.

About croup

Croup (laryngotracheobronchitis) is caused by viruses, most often parainfluenza virus, that infect the upper airways. The voice box and windpipe become inflamed and swollen. In young children the airway is already narrow, so even a small swelling causes a hoarse voice, a barking cough and a noisy sound when breathing in, called stridor. Symptoms usually begin after one to three days of a runny nose and low fever and are worse at night.

Most croup is mild and can be managed at home with calm and comfort, because crying and distress make the airway noises worse. Moderate or severe croup is treated with a single dose of a steroid (dexamethasone) and, in severe cases, adrenaline by nebuliser, in hospital. These treatments work quickly and safely and reduce the need for admission.

Other conditions can mimic croup and are more serious: epiglottitis (now rare with Hib vaccination), a foreign body in the airway, bacterial tracheitis and allergic swelling. Any child with severe breathing difficulty, drooling, difficulty swallowing or who looks very unwell should be taken to hospital at once.

Common symptoms of croup

Symptoms vary from person to person. Common ones include:

  • A harsh, barking cough, often described as sounding like a seal
  • A hoarse voice or cry
  • A high-pitched, noisy sound when breathing in (stridor), which gets worse with crying or activity
  • A runny nose and low-grade fever for a day or two before
  • Symptoms that are worse at night and improve in the morning
  • A child who is irritable and unsettled but alert

Causes and risk factors of croup

There is rarely a single cause. Factors that may contribute include:

  • Viral infection, most often parainfluenza virus; also influenza, RSV and others
  • Children aged 6 months to 3 years, whose airways are narrow
  • Autumn and winter months, and exposure to children with colds
  • Not caused by cold air itself, although cool night air is often soothing
  • Allergy and acid reflux may make some children more prone to repeated croup

When to see a doctor

Most children with mild croup recover at home, but you should call a doctor if the cough is troublesome. Go to hospital immediately, or call an ambulance, if your child has:

  • Noisy breathing (stridor) when resting or calm, not only when crying
  • Skin sucking in at the neck, above the breastbone or between the ribs when breathing
  • Blue or grey lips or face, or severe paleness
  • Drooling, being unable to swallow, or sitting up leaning forward with a very sore throat
  • Extreme tiredness, drowsiness, restlessness or distress that does not settle
  • A child younger than six months with croup, or a child with a history of airway problems

Homeopathic medicines for croup

The medicines below are commonly considered in homeopathy for croup, with the symptom picture traditionally associated with each. They are listed for general information, not as a prescription.

(1) Aconite napellus: Sudden onset, often after cold dry wind, with a dry, barking cough, fear, restlessness and anxiety in the first night of croup, before any catarrh.

(2) Spongia tosta: Dry, barking, “sawing” cough like a seal with a hoarse voice, a dry, tight airway, and breathing that sounds like a saw through wood, worse before midnight.

(3) Hepar sulphuris calcareum: Croup with a loose, rattling cough after the dry stage, with chilliness, great sensitivity to cold air and irritability, and cough worse on uncovering.

(4) Belladonna: Croup with sudden fever, a hot, flushed face, dry, spasmodic cough and a sore, sensitive larynx, worse around 3 a.m.

(5) Bromium: Croup with a hoarse, whistling and wheezing cough, a rattling breathing and a tendency to cough and wheeze in children with a sensitivity to dust and damp.

(6) Kali bichromicum: Croup with stringy, sticky mucus that is hard to bring up, with a ringing, brassy cough and a tendency to relapse.

(7) Phosphorus: Hoarse, tickling cough with a tight chest, a burning sensation in the throat and thirst for cold drinks, worse from talking, laughter and cold air.

(8) Sambucus nigra: Sudden attacks of suffocation at night, with the child waking up suddenly, a dry, hot blocked nose and a spasmodic cough, with blue face.

(9) Drosera rotundifolia: Deep, barking, spasmodic cough in bouts, with gagging, worse after midnight, and the child holds the chest during coughing.

(10) Iodum: Croup with a hoarse, dry cough and a hot, restless child with a ravenous appetite, who is better in the open air.

Homeopathic treatment of croup at Welling Homeopathy

These are only basic indications – many more homeopathic medicines are used for croup, and the right one is chosen after detailed case-taking by an experienced doctor.

Homeopathy is offered as supportive care for the cough of croup. Reliable scientific evidence that homeopathic medicines treat croup is lacking, and croup can narrow the airway quickly. A child with noisy breathing at rest, chest retraction or blue lips needs emergency medical treatment, not home remedies.

Self-care and lifestyle tips

These measures support general health and may ease symptoms, but they do not replace treatment when it is needed.

  • Stay calm and hold your child upright and close, as a calm child breathes more easily; crying makes croup worse.
  • Sit with your child in a steamy bathroom for a few minutes or take them outside into cool night air, which many parents find soothing, though proof of benefit is limited.
  • Offer small, frequent sips of warm fluids, and breast or bottle feeds, to prevent dehydration.
  • Raise the head of the cot or mattress slightly and keep the room at a comfortable temperature; avoid smoke.
  • Give paracetamol for fever or discomfort as directed for age.
  • Do not give cough syrups or suppressants to young children unless a doctor prescribes them.
  • Stay close through the night; if your child’s breathing becomes noisy at rest, call a doctor or go to the emergency department without delay.

Frequently asked questions about croup

1. What does croup sound like?

A harsh barking cough like a seal, a hoarse voice and a high-pitched, squeaky sound when breathing in (stridor).

2. Is croup dangerous?

Most cases are mild, but a small number of children develop severe airway narrowing and need urgent treatment. Noisy breathing at rest, chest retractions, blue lips or drooling are emergencies.

3. Does steam help croup?

Many parents find it comforting, but studies have not shown clear benefit. Calming the child and, if needed, a steroid dose from a doctor are the effective measures.

4. Why is croup worse at night?

Airway swelling often increases at night, and children are lying down and may be upset when waking. Symptoms usually improve during the day.

5. Can homeopathy treat croup?

Homeopathic medicines are traditionally used for croupy coughs as supportive care, but reliable scientific evidence is limited. Seek urgent medical help if breathing is noisy at rest or the child struggles to breathe.

6. How long is a child with croup contagious?

As long as the cold symptoms and fever last, usually three to five days. Children may return to school when they are well and the fever has gone.

7. Can croup keep coming back?

Some children get croup repeatedly until about the age of six, when the airway grows larger. Frequent croup should be assessed by a doctor for allergy, reflux or airway problems.
