Polycystic ovarian syndrome (PCOS) is one of the most common hormonal conditions in women of reproductive age, and one of the leading causes of difficulty in conceiving. Many women discover they have it only when they try to become pregnant.
What is PCOS?
In PCOS the ovaries do not release an egg regularly, and the body makes more male-type hormones (androgens) than usual. Doctors usually diagnose it when at least two of three features are present: irregular or absent ovulation, signs or blood tests of raised androgens, and polycystic-appearing ovaries on ultrasound. The “cysts” are immature follicles that did not mature enough to release an egg.
Common signs and symptoms
- Irregular, infrequent or absent periods
- Acne, oily skin and excess facial or body hair
- Thinning of scalp hair
- Weight gain, especially around the waist, and darkening of skin folds
- Difficulty becoming pregnant
Some women with PCOS have normal monthly cycles, so the diagnosis is not always obvious.
Insulin resistance, diet and lifestyle
Many women with PCOS have insulin resistance, which pushes the ovaries to produce more androgens. A diet low in refined sugar and high in fibre, regular exercise, adequate sleep and even a modest weight loss can improve cycles and ovulation.

PCOS is linked with a higher risk of type 2 diabetes, high cholesterol, high blood pressure, fatty liver, sleep apnoea, depression and thickening of the womb lining, so regular health checks are important.
Treatment
