Infertility is usually defined as not conceiving after a year of regular, unprotected intercourse (six months if the woman is over 35), or as repeated miscarriages. Roughly a third of cases are mainly due to female factors, a third to male factors, some to a combination of both, and in a minority no cause is found.
Why finding the cause matters
Treatment works best when it targets the actual problem. Starting medicines or procedures without knowing whether the issue is ovulation, the tubes, the womb or the sperm can waste months, especially when age is a factor.
Common female causes
- Ovulation disorders, such as PCOS, thyroid disease or raised prolactin
- Blocked or damaged fallopian tubes, often after infection or surgery
- Endometriosis
- Fibroids, polyps or an abnormally shaped womb
- Reduced ovarian reserve with age
Common male causes
- Low sperm count, poor motility or abnormal shape
- Varicocele (enlarged veins in the scrotum)
- Infections, hormonal problems or earlier surgery
- Smoking, alcohol, heat exposure, obesity and some medicines
Basic investigations
A semen analysis for the man, and for the woman blood tests for hormones including thyroid and AMH, ovulation tracking, a pelvic ultrasound, and a test of tubal patency such as HSG. Both partners should be assessed together. Homeopathic treatment is individualised and can be considered once the picture is clear.
