Luteinized unruptured follicle syndrome (LUFS), also called trapped egg syndrome, is a condition in which an egg develops normally in the follicle and the hormones rise as they should, but the follicle does not rupture and the egg is never released. Because periods, hormones and temperature charts can look normal, it is a silent problem that is easy to miss in women who are not conceiving. This page explains what LUFS is, its causes, why routine tests miss it, how it is diagnosed, the conventional options, and how homeopathic treatment of LUFS is offered at Welling Homeopathy in Mumbai and online.

LUFS at a glance
- What it is: an egg develops and the follicle turns into a corpus luteum, but the follicle does not rupture, so no egg is released (trapped egg syndrome).
- Symptoms: usually none; periods look regular, which is why it is a silent cause of infertility.
- Linked with: endometriosis, PCOS, pelvic inflammatory disease, fertility drugs and regular use of anti-inflammatory painkillers around ovulation.
- Diagnosis: serial transvaginal ultrasound scans through the cycle; hormone tests and temperature charts cannot show it.
- Treatment: treat the associated cause; ovulation triggers, IUI or IVF may be advised; homeopathic care is supportive.
What is luteinized unruptured follicle syndrome?
In normal fertile women, ovulation, the rupture of the mature ovarian follicle and the release of the egg, occurs about 36 to 38 hours after the surge in luteinising hormone (LH). In LUFS, the egg develops quite normally within the follicle, and all the hormonal changes take place reasonably normally, but the follicle never ruptures and the egg is never released. The follicular surface fails to dissolve even though it is stimulated by an LH spike, and the follicle continues to turn into a corpus luteum as expected.
Because the corpus luteum makes progesterone, the basal body temperature rises, blood hormone levels look normal and even an endometrial biopsy may suggest that ovulation has occurred. LUFS is therefore hard to spot and easy to miss.
LUFS is also called trapped egg syndrome. It can occur occasionally in normal cycles; when it happens repeatedly it can reduce the chance of pregnancy.
Symptoms of LUFS
LUFS does not show any signs and symptoms. It is a silent problem, and the diagnosis is easy to miss. Clues that make doctors look for it include:
- Difficulty conceiving despite regular cycles and normal hormone reports
- Normal progesterone and a rise in basal body temperature, but no pregnancy
- Associated conditions such as endometriosis, PCOS or a history of pelvic infection
- Pain in the middle of the cycle that is absent, or absence of the usual ovulation signs
- Unexplained infertility after normal tubal and semen tests
Causes and associations
- Trapped egg syndrome or LUFS seems to occur more frequently in women taking fertility drugs
- Women who have had pelvic inflammatory disease
- It is commonly seen in patients with PCOD (PCOS) and endometriosis
- Regular use of prostaglandin inhibitors such as ibuprofen, diclofenac and other anti-inflammatory painkillers around ovulation, which can block follicle rupture
- Pelvic adhesions and other causes of reduced ovarian mobility
Why routine tests can miss LUFS
Ovulation is determined by hormones produced during the menstrual cycle. They do not show whether the actual release of the egg from the ovary has taken place. They assume that if the hormones are produced in proper amounts then ovulation has occurred. The hormones and the physical release are two separate events.
| Test | Result in LUFS | Does it show release of the egg? |
|---|---|---|
| Basal body temperature chart | Rises as normal | No |
| Mid-luteal progesterone | Normal | No |
| LH surge (ovulation kit) | Positive | No |
| Endometrial biopsy | Suggests ovulation | No |
| Serial transvaginal ultrasound | Follicle persists and does not collapse; little free fluid in the pelvis afterwards | Yes: this is how it is diagnosed |
How LUFS is diagnosed
Diagnosis is difficult. It can be made only by serial vaginal ultrasound scans to track ovulation, repeated over several days around the expected time, to see whether the follicle collapses and whether free fluid appears in the pelvis afterwards. A single scan is not enough, and because LUFS can occur sporadically, the finding is usually confirmed in more than one cycle.
Conventional management
- Treat the associated cause: endometriosis, PCOS or pelvic infection
- Stop regular anti-inflammatory painkillers around ovulation, if you are trying to conceive and your doctor agrees
- A medicine to trigger ovulation (hCG injection) at the right time, under scan monitoring
- IUI with ovulation induction, or IVF, which bypasses the need for the follicle to rupture naturally
- Time and repeat cycles, since LUFS can resolve on its own

Frequently asked questions about luteinized unruptured follicle syndrome
1. What is luteinized unruptured follicle syndrome?
LUFS is a condition in which the egg develops in the follicle but the follicle does not rupture, so the egg is not released. It is also called trapped egg syndrome.
2. What are the symptoms of LUFS?
There are usually none. Periods can be regular and hormone tests normal, so it is easy to miss.
3. How is LUFS diagnosed?
Only by serial transvaginal ultrasound scans that track the follicle through the cycle. Hormone tests and temperature charts cannot detect it.
4. What causes LUFS?
It is more common in women with endometriosis, PCOS, a history of pelvic infection, those taking fertility drugs and those using anti-inflammatory painkillers around ovulation.
5. Can I get pregnant with LUFS?
Yes. LUFS may occur only in some cycles, and treatment of the cause, ovulation triggers, IUI or IVF can help. Welling Homeopathy’s clinical experience is described above.
6. Is there a treatment for LUFS in conventional medicine?
Options include treating the cause, a trigger injection, IUI or IVF. Homeopathic care is offered as a natural, supportive approach.
7. Can LUFS be cured?
It can be sporadic and may resolve on its own. At Welling Homeopathy, our experience is that many women conceive naturally; results vary from person to person.
8. Do painkillers cause LUFS?
Regular use of prostaglandin inhibitors such as ibuprofen and diclofenac around ovulation can block follicle rupture, so discuss painkiller use with your doctor when trying to conceive.
9. Where can I get LUFS treatment in Mumbai?
Welling Homeopathy sees couples at its clinics in Andheri East, Matunga and Thane West, and online. Your first consultation and assessment is free.
